Severe Vibration at slow speeds

I have a 2003 1500 hemi 5.7 4x4 when I take off from a light or a stop sign I get a Vibration so bad it shakes the whole truck. I had carx look at it and they said I need a new rear end when they took the drive shaft off and turned the rearend by hand they said they can feel ruffness. Would it really be the rear end.

It could be possible but I'd get a 2nd opinion as a rear end on a vehicle can be expensive even if you swap it or even rebuild it yourself.

Was this shop you took it to a driveline, transmission TYPE shop or just a general shop?

Sometimes if it's a pinion bearing and hasn't went completely south you may get by replacing the bearing, race and seal and flushing the rear differential oil which if you don't know the history you can flush the rear differential.

If you want to see if you have shavings in the rear differential yourself or even learn how to check U-Joint or drivetrain let YouTube be your friend.

There's a YouTube channel called 1road or OneRoad, can't remember which way he spells it but he's got a video about changing the rear differential fluid and he's a rookie. That would be a good start if you suspect or want to check or flush it yourself.
