front control module

dose anyone have the part # for a front control module [fcm] for a 04 dodge ram 1500 5.7 hemi
my passenger headlight dosent work, then 2 days later my blinkers, and wipers stop working. |

fixed the headlight problem, it was a ground wire that lost its ground, located by the air filter box
the wiper, blinker, highbeam problem was the connector for the blinker/wiper swicth. i had the truck at the dealer for AC warranty work and they didnt plug it all the way in, it just so happened that everything went out at once. |
