Rear End Growl??

I have a growling/rumble sound while under light pulling with or without the Trailer on. If I let off the trotle it's gone. I mostly noticed it in 5th gear . It was very minor a year ago and I only noticed it if I listened for it. I recently went on a road trip with the 4 horse trailer and now the sound is more pronounced and in 4,5&6th gear. If I pull hard the sound is gone, might be drowned out by the cummins, The truck is a 1 ton 4X4 2001 dually 6 spd standard with 270,000 km on it. Is this the rear end giving out. How do I find where the noise/problem is. Should I start with opening the rear end first and go from there. If it is the rear end what cost and how hard is it to do yourself?
I am fairly handy, just replaced the vp44 but I have never worked on a rear end or tranny
