We need more info on your van.....fullsize, delivery(Sprinter), or mini?
What year, and what engine, etc.

Don't be upset if a moderator moves it to the van section if it is a full size or a Sprinter.
We have some really sharp members there with a ton more time and experience with the B series than I do.

Have you checked the engine codes yet? A flip of the key without turning on the engine, on-off-on-off-on should show something.

Maybe the van has an emissons leak that happens when the gas tank is down to 1/4 full. I would check the return line and canister.

Maybe the straw in the gas tank doesn't reach down that far to get the gas. lol, just kidding.

You had mentioned that you have a 1994 model. A 3.9L engine should be in a B2500. However, a B250 should have the inline slant 6, 225cid, 3.7L engine.
