99 Ram 1500 Regency conversion van, 360, dual a/c, dual heater cores.

How big a deal is it to replace the front heater core? A casual looking
around makes me think it can be done from the engine side alone, with no
dash pulling required. If I have to pull the glove box, that's not a big
deal.

Anyone done it? What does a new core cost these days?

I have a '97 model, its a bit different then yours.
But might be almost the same. It was not to difficult to change this thing, but I had to remove the gas of the a/c and to refill it after wards. From inside there where only 2 screws behind the clove box and a few under the dashboard on the passenger side. No removel of the dash board on 97 models.

Just as Bavariavan said, you should be able to remove the heater core/evaporator housing from the front. However my knowledge of 98-2003 vans is VERY limited.
