could anybody tell me how to tell if the pump is bad.

In many cases, the motor will run rough if it starts at all. If it does start, it'll kick out alot of unburned fuel out the exhaust after it's warmed up.
The trouble code P0216 is an indicator of trouble and P1688 is generally held as the injector pump death code.
The '99's have to be checked with a scan tool.

The 98.5's and '99's can suffer from internal pump problems when turned up with a module. The 2000-2002 pumps didn't suffer that.

I think all rebuilt pumps are upgraded with more durable heat resistant electronic boards.
From what I was shown at a rebuild shop, heat to the electronics is the #1 killer with rust damage from lack of fuel treatments was #2.
That's why we like a fuel pressure gauge to monitor lift pump pressure and use fuel treatments. Given the hot summertime temps here in Tennessee, that's why I run a fuel cooler too.
