97 3.0 Voyager misses/shutters after warmup

97 3.0 Voyager runs fine from a cold start, but after restarting while the engine is still warm, it misses/shutters badly. Plugs, wires, Cap and button are less that 12 months old. I disconnected the battey for about 2 hours ard then drove it about 15 miles. It ran fine until I turned off and restarted it, missing/shuttering again. Then I gathered the codes:

Code 12-Direct battery input to PCM was disconnected within the last 50 Key-On cycles.

Code 27-Fuel injector control circuit-fuel injector output driver does not respond properly to the control signal.

Code 21-Oxygen sensor signal-neither rich or lean condition is detected from the oxygen sensor input.

Code 55-Completion of fault code display on Check Engine lamp.

Kinda don't know where to go from here. Anyone else had a similar problem?
