I have a 1994 3.8L Grand Caravan LE. I have had some problems where it doesn't seem like it wants to stay in gear. I'll give it gas on the highway and it'll sputter. It doesn't do this all the time and some times it's worse than others. I just had a transmission put in about 2.5 yrs ago (40,000 miles). Any ideas would be appreciated. When I check the codes, I get a 12 & 21.

It doesn't switch gears like it normally does and seems to get stuck in between them. When I give it more gas, it just doesn't want to go. This seems to happen the most whenever I fill up my tank. It's been running near empty for quite some time and whenever I fill it up, that's when it seems I have the most problems. It also idles lower than normal. I've got to get another year out of this van so any help would be GREATLY appreciated.
