class V hitch?

Does anyone know where to purchase one...

preferably a bolt and weld-on type hitch.. I've got a homemade hitch thats been on my truck, but the person who made it did a terrible job... Its not lined up directly in the center, and it is welded to my bumper (dont like)... and I think its around a class III-IV hitch... What companies make Class V hitches for our trucks?

I bought a hitch for my B2500 and believe it or not, the best price I found at the time was at one of those big 'U-Haul' centers....

I really forget what I paid for it though, it's been about 9 years ago...
